REGISTERED NURSES (RN)/LPN
Registered professional nurses or licensed practical nurses must maintain knowledge of current nursing standards and establish priority in provisional care to the clients.
A Registered Nurse (RN)/LPN of SCHC provides services on an hourly or per visit basis to meet whatever professional skilled nursing care is required. A Registered Nurse also provides the following:
Regularly assesses the nursing needs of clients.
Participate in the establishment and implementation of client’s plan in coordination with case manager and conduct face-to-face initial assessment of the client at client’s home of residence.
Provide nursing services as needed in accordance with the client’s comprehensive care or service plan.
Report problems and progress of the clients to the supervisory personnel or to client’s personal physician.

A CNA assists clients with personal care such as bathing, feeding, toileting, grooming, shaving, dental care, dressing, and taking vital signs as well as other medically related tasks. They also report problems and progress of the clients to the supervisory personnel.

SCOPE OF SERVICES / TYPES OF CLIENTS SERVED
SCHC offers a comprehensive private home care service to clients who are not capable of functioning independently on their own and requires professional care services and watchful medical oversight.
SCHC is currently providing personal support, companion, and sitter services to clients who are functionally impaired, assisting them with activities of daily living. These services are provided at the client’s home by person(s) licensed as a Certified Nursing Assistant (CNA) and supervised by a licensed Registered Nurse (RN) in the state of Georgia.
Provides services to the following clients:
A person who has been discharged from the hospital but not fully recovered or has a condition(s) that need monitoring.
Someone who is in the final stages of an incurable disease.
A person categorized as a mentally challenged individual.
A person with a chronic condition and requires care until he/she has stabilized (e.g. Heart/Lung diseases, diabetes, etc.)
